Reimbursement request  - Submit if you used your own payment for a church purchase

In Kind Donation-  Submit if you gave physical items to the church

Debit Card Purchase – Submit if you used a church debit card for a church purchase

Check Request – Outreach and LB should use this form when requesting Good Samaritan assistance checks to be cut and sent out.

Release from Restriction – Submit if funds from a restricted fund (property, benevolence, etc) need to be released to cover a purchase

Invoice – When Mt Zion needs to invoice a group or company

Transfer request – Transfer funds from financial institutions (officers only)
